Tech Help Needed for Online Survey signup sheet

We are looking for a tech-savvy person to help us provide an online option for our Commitment Sunday Cards. We are looking for someone who can provide their expertise and labor in taking our physical (hard-copy) commitment card and making it into an online survey that can then produce a spreadsheet for us. Ideally the online survey would have a mobile phone compatible option. Volunteer can be an adult or teen.
