Chiropractor Maintenance Visit, Wednesday, 3/4 signup sheet

Sign up for a maintenance appointment with Dr. Dustin Zimpfer,our chiropractor!

If you are signing up for your first visit, please sign up for two consecutive appointments due to your evaluation appointment taking longer. Bring a copy of the front and back of your insurance card to your first appointment.

The cost of this visit depends on your insurance. If you are NOT on RevLocal's insurance plan, please call the Integrated Health Chiropractic office to get a quote on the cost of your visits (740) 920-4749. Payment must be made at the time of your appointment. They accept card, check, or exact change in cash. If you were unable to sign up, there will be opportunities every Wednesday.
