COVID 19 VACCINE CLINIC AT WESTGATE PARK CITY RESORT & SPA signup sheet

THIS IS A NO COST VACCINE CLINIC!

Vaccine will be administered by a registered nurse via Community Nursing Services (CNS). The vaccine will be the Moderna vaccine, which requires two shots. (We will sign you up for shot #2 when you arrive on June 1st.)

June 1st for shot #1
June 29th for shot #2

PLEASE BRING THE ITEMS BELOW TO YOUR APPOINTMENT:
--Insurance Card (if you have insurance)
--Driver’s License (if you don’t have insurance)

Snacks and beverages will be provided. Please arrive 5 minutes before your appointment time. The clinic will be located in the Bear Conference Rooms.

THE HARD DEADLINE FOR SIGNING UP IS MAY 21ST. WE MUST TURN IN THE FINAL HEAD COUNT TO Community Nursing Services (CNS) BY May 21st.

They will only bring the exact number of shots for those who sign up with no extras. It is extremely important that you commit to your appointment times by May 21st or cancel if you will not be able to attend. We do not want any vaccines to be wasted.
