Augsburg Central Health Commons signup sheet

Hello,

Thank you so much for your willingness to help at the Augsburg Central Health Commons! We are so excited to have you help us continue to serve those who are experiencing homelessness or who are marginalized housed in Minneapolis. Our shifts are from 9:30am-12:00pm.

For those of you in the graduate programs, you can count viewing this voice thread towards total practicum hours.

COVID REQUIREMENTS:

BEFORE you come to the health commons
1.) Please review and sign this liability waiver. https://forms.gle/UmxY4uHxMTiLFukC7
2.) On the day of your experience, you will need to complete the Campus Clear app daily form. Although we are not technically going to be on campus, we do need to fill out the form.

Central Lutheran Church: 333 S.12th St. Minneapolis, MN 55404

We are across from the convention center in downtown Minneapolis. Please park in the CHURCH parking lot (NOT the Convention Center ramp). Please bring your ticket in for validation for free parking.
