2019 Adopt-A-Family signup sheet

Thank you for participating in the Annual Adopt-a-Family  at Meadowbrook Middle School! Our goal every year is to make sure each and every participant is adopted. Thank you for helping us reach this goal. Donations must be received by December 4, 2019 to allow time for preparation and distribution of gifts. If possible, please provide a gift receipt for clothes and shoes.

Packaging Guidelines:
 Make sure gifts are unwrapped – rolls of wrapping paper are appreciated.  Label each item (e.g. Family A01 , boy, age 8).  Place each family member's gifts in separate labeled bags.  Place all bags inside a large bag or box and tag with Family ID Number.  Note the dollar amount on gift cards and place them, with the receipts, in an envelope.
